You could imagine that you would be satisfied that you have reached the pinnacle of your career. A life of compliments and comfort is waiting for you. Now you are happy with your fate.

But that often this is not what happens. Sir Bradley Wiggins won Olympic gold medals on the track at the Athens, Beijing and Rio de Janeiro competitions, and also won the road time process in London 2012, two weeks after he became the first British rider who won the Tour de France.

For 15 years he was at the height of his sport, a celebrated national treasure and bicycle icon. He was named CBE in 2009 and knighted in 2013.

But in an interview last week he confessed that he became seriously addicted to cocaine in the years after his career in 2016, so that he risked not only his reputation but also his life.

“There were times when my son thought I would be found dead in the morning,” he said. “I was a functioning addict. People would not realize – I was usually high for many years. ‘

You may think that this is unusual, but I can assure you, there are many more ‘functioning’ addicts than you would propose, especially when it comes to cocaine and especially among those you would otherwise think that you are successful. They are hidden in sight.

While other drugs such as alcohol or opiates have demanding signs – the scent, sedation or slow movement – it is more difficult to recognize when people have used cocaine.

They will usually develop a usage pattern that will minimize the opportunities to be discovered, to take the medicine at certain times, or have routines that mean that they can use without affecting their most important tasks and responsibilities.

Of course I use the term ‘function’ with reserved. In many ways it is misleading and they do not work. Yes, they can hold a job, have a family and appear respectable and happy. But on a deeper, more meaningful level it all went wrong.

Functioning is not about going through the movements of life. It is fascinating every day with the world around you and the people in it. Cocaine users anestheter themselves, live behind a barrier and step psychologically away from the people closest to them.

What we really mean by ‘functioning’ is that they exist in society and exist unnoticed. These are the addicts who manage to hide their addiction better than others.

And what that in turn means is that the addict can often fool himself that they are good. Their life is not implanted. They have not lost their job or destroyed their health, at least not clear, and so are they certainly one of the winners of life?

The reality is a ‘functioning’ addict in just as much torment, just as much worries, as a ‘non-functioning’ addict. And the fact that they have not all lost it is purely a matter of happiness.

In more cases than not, these people with a cord, a small gust of wind, walk away from crashing on the floor. Sometimes they slide away and they are caught red -handed to take drugs, but that is not the most likely cause of their downfall. More often it is an unexpected incident that gives the balance: a mourning, a rocky patch in their relationship, a difficult time at work.

Then they suddenly have to use more medicines to numb themselves what happens and get through the day. They start to use at work or at family events. People notice it, and those carefully composed routine that meant that they could hold it all together. The steady downward spiral starts.

I once had a patient who had used cocaine daily for nine years. Everyone around her was not at all consciously until her husband lost his job and put it a tension on their marriage. She started to use more and suddenly, for the first time in almost a decade, the mask slipped and people started to note that she was acting strangely.

Sometimes she stayed up for days and of course her family and colleagues became suspicious on time. She told me that she felt like a dancer who could not hear the orchestra what she had to dance on. But she had learned the steps so well, nobody who looked at her would ever know.

When her marriage started to struggle, it was as if a new conductor had taken control of the orchestra and had changed the pace very slightly. Suddenly everyone could see that she only pretended to danced on the tune that the orchestra played.

That is quite beautiful the tragedy of so -called functioning addicts for me. There they are, because of the movements of life, but not really life, waiting for the pace to change and it all crashes.

Many people who are victims of violent crime will understand this – it is a form of hypervigilance where your brain constantly scanning for threats. It can be incredibly debilitating. I was particularly impressed by her efforts to pass with therapy. Today, she said, she is in a place where she can forgive the people who are accused of having taken her hostage. She is also lucky that she can of course afford therapy – many others can’t.

It is upset to think that there are other victims who would benefit from this kind of help, but just can’t get it.

The Royal College of Psychiatrists has expressed its opposition against the supervised bill in England and Wales about ‘Many, many factors’. This is important because, according to the current provisions of the bill, a panel with a psychiatrist would supervise assisted dying issues.

I share the concerns of the college. Although in principle I do not object to assisted dying, and understand the arguments before, I am on my guard for the lack of security. It is currently open to abuse and coercion. Let’s not make any bones about it, we are talking about doctors who kill patients. It feels rushed and badly thought out.
