Often the first signs of a migraine visual disruptions such as blind spots, tingling in the hands and face, weakness and changes in speech – known as the ‘aura’ phase.

But there is also a less well-known period, the pre- and prodromal phase, with its own set of debilitating symptoms-including brain fog and fatigue, mood changes (irritability and anxiety), neck pain, dizziness, more urine and yawning.

It can also cause thirst and crazy for food (of which the patient then wrongly thinks it is the ‘trigger’ for the migraine itself).

This prodromal stage can occur hours or even days before the headache – but while the symptoms appear before the aura, they can also overlap.

Now scientists have discovered at King’s College London that a medicine that is already known to relieve migraine pain can also stop these early symptoms, which can affect up to 80 percent of migraine patients.

The medicine, called UbrogePant, is the first to be demonstrated to help patients at this stage.

For the study, published in the journal Nature Medicine, 438 participants aged 18 to 75 who had had migraine for at least a year, were randomly assigned to one of the two groups.

They took a single 100 mg dose of Ubrogepant or a placebo when their former symptoms struck. They were then asked to repeat this process by using the other treatment at least seven days later.

The participants passed fewer symptoms when they used UbrogePant. For example, two or three hours after taking UbrogePant, for example, they had twice as much chance of neck.

They also reported that they could concentrate only an hour later and experienced less sensitivity to light.

There were some side effects, such as nausea, sleepiness and dizziness, but these were relatively unusual and not considered serious.

UbroGepant works by blocking calcitonin-gen-related peptide (CGRP), a chemical messenger released by certain nerves during a migraine involved in the transfer of pain signals.

A study by Umea University in Sweden, published in 2015 in the magazine Cephalalgia, showed that 77 percent of people with migraine experienced front tires – where women have more than men (other studies have the prevalence on the 30 and 80 percent of the migraine patients).

Dr. Nicholas Silver, a consultant – neurologist at the Walton Center in Liverpool, said: ‘This is a really interesting and useful study – the Prodromal Stadium of Migraine is important because these symptoms are often relatively invisible but very invalidate.

‘They also allow a person to detect when a migraine starts.

“This study provides good evidence that treatment with CGRP antagonist -therapy early in a migraine attack, indeed before the headache, has a good use to reduce these symptoms.”

He adds that this type of medicine is probably the first acute attack treatment class that is unlikely to cause a headache rebound or medication. (Scientists do not know exactly why, but some painkillers can cause more pain when they are often used.)

This concept would mean that these drugs can be prescribed more broadly and not only to be stored for serious attacks.

Although UbrogePant is available in various countries, including the US and Canada, the manufacturer Abbvie (who has financed the newest study and partially designed) has not planned to bring treatment to the UK at the moment.

However, there is another type of CGRP receptorantagonist, Rimeege, available here.

“Although not proven as UbrogePant in this study, we believe that it can be used safely in aura or in the prodromal phase,” says Dr. Silver.

There are other treatments underway – scientists in the US, for example, have developed a skin plaster that can be more effective than migraine pills.

Taking them as tablets is often not effective if the patient then vomits because of the migraine.
